2019 Member Survey

To help your association and our coaches do a better job, we need to hear from you.

Please take a few minutes to share your thoughts and to sit down and collect the thoughts of your player(s).

The survey is designed to be filled out once to cover all your players, however, we would encourage each parent to complete the survey separately. In other words, the parent who sits down with the player(s) to collect the player’s information can complete the survey one time. Then the other parent can complete the survey a second time and just skip the For Players Only section.

Note. The survey is anonymous but there is a field at the end where you can record your name and contact information if you are comfortable with someone from the association contacting you to collect more information.
